Berks Community Health Center adds four new providers

​Berks Community Health Center (BCHC) added four new providers in the second half of 2025. Each of these providers brings valuable experience and compassionate care to the BCHC team, and their positive impact is already being felt by the patients they serve.

Emma Cooper, MSN, CPNP-PC, joined the pediatric team and Manifay Sesay, FNP-BC, joined the primary care team at BCHC’s Rockland location. BCHC’s 838 Penn St. location added Madu Gabriel, CRNP-FNP-C, and Angelina Menard, FNP-C, to the primary care team.

Emma earned her Master of Science in Nursing from Villanova University in May 2025. Her clinical training included pediatric primary care at Sunshine Pediatrics in Conshohocken and Sunnybrook Children’s Hospital of Philadelphia Primary Care in Pottstown, along with additional rotations in adolescent medicine, behavioral health, and orthopedics. She is an active member of the National Association of Pediatric Nurse Practitioners and has a passion for providing exceptional care to pediatric patients.

Manifay Sesay returns to BCHC after serving as Clinical Nurse Manager from 2019-2021. She has experience as a Registered Nurse at Genesis Healthcare & Penn State Health-St Joseph Hospital, with vast experience in home health care, geriatric care and in mental health working through various local nursing agencies. Manifay completed her Master of Science in Nursing at the Pennsylvania College of Health Science in 2024. Beyond her clinical expertise, Manifay is deeply involved in community service. As a former Vice President of the Sierra Leone Association of Reading, she has led humanitarian initiatives supporting food distribution, medical supply donations, and disaster relief efforts locally and abroad. Both her professional work and personal life demonstrate a deep commitment to compassion and community service.

​Madu Gabriel completed his practicum experience with BCHC in 2024. He previously served as Administrator and Director of Nursing for a home health agency in the area, and as a Clinical Nurse at Hershey Medical Center. Madu is an active member of the Nurse Practitioner of Central PA and the current President of Nigerian Nursing Association Pennsylvania Chapter (NNAPAC) part of National Association of Nigerian Nurses in North America. He is also the founder of the Hopee Foundation, International, a non-profit dedicated to supporting widows, orphans, and underserved populations globally.

Angelina Menard earned her Master of Science in Nursing as a Family Nurse Practitioner from Chamberlain University, after completing earlier degrees at Alvernia University and the Reading Hospital School of Health Science. She brings extensive clinical and leadership experience to BCHC, having previously served as a Family Nurse Practitioner at Berks Genesis Family Medicine and Matrix Medical Network, and as Director of Nursing at CareSense Home Health Care. With a deep love for the highly diverse and culturally rich population of Berks County, Angelina is committed to uplifting local health care and ensuring access to high-quality, comprehensive services. She is passionate about partnering with patients to manage both simple and complex conditions, empowering them to achieve and maintain optimal health.
